Understanding Your Coverage and Costs

Purpose

Where to see what you are enrolled in, what it costs you each paycheck, what your employer pays, and where the related documents live.

Your Benefits Dashboard

The Benefits tab shows your current elections: each plan, who is covered under it, and what it costs.

Two numbers matter for every plan:

  • Your cost is the amount taken from your paycheck.
  • Employer contribution is what your company pays toward the same plan. It does not reduce your pay. It is the part of the premium you are never billed for.

Your Enrollment History, reached from the Quick Link on your home page, keeps your elections from previous years.

Where the money shows up

Your share appears as deductions on your pay stub, and your employer's share appears under employer contributions. See Understanding Your Pay Stub.

If a deduction is not what you expected, the usual causes are:

  • A new plan year, with new rates.
  • A mid-year life status change.
  • A change of coverage tier, for example adding a dependent.
  • A pay period where the deduction was caught up or skipped.

Compare the deduction on the stub with your cost on the Benefits Dashboard. If the two disagree, ask your HR team before assuming the stub is wrong, since the correction has to be made on the benefits side.

Documents and cards

  • Insurance cards are issued by your carriers and sent to you directly. UZIO does not produce, distribute or store them. If a card has not arrived, contact the carrier or your HR team.
  • Claims are handled by your carrier. UZIO holds no claim information.
  • Plan documents your employer has shared are normally with your other documents under My Info. If you cannot find a summary of benefits or a plan booklet there, ask your HR team for it.

On the mobile app

Enrollment and any change to your elections are done in the employee web portal. Whether the app shows your current benefits is being confirmed, so use the web portal when you need to be sure of what you are covered for. See Using the UZIO Mobile App.

  • What a plan covers, what a claim paid, network questions: your carrier.
  • Which plans your company offers, eligibility, rates, enrollment windows: your company's HR or benefits team, or its broker.
  • A cost or deduction that does not match your election: your HR team.
